Strengthening Families
As our Community Impact model takes shape, we will be collecting results in our three impact areas and reporting back to our communities and stakeholders. The case study information below gives an indication of baseline data we’ve already collected as we begin this process.
Strengthening Families: Families and individuals have access to services and resources that support self-sufficiency and healthy living.
Access to Healthcare for Seniors
Situation:
- Iowa leads the nation in the proportion of older residents. Several counties within the UWECI service area exceed the nations average percentage of older adults.
- Home and Community Based services such as home delivered meals, in-home health, and transportation can assist individuals in maintaining their independence and can be provided at approximately 25% the cost of nursing home care.
- In Iowa 286,000 family caregivers provided 306 million hours of care for an annual market value of over 2 million dollars.
Response:
- In fiscal year 2006, approximately 26,500* seniors were provided care and support services by United Way partner agencies.
- 900 seniors in the UWECI service area are receiving case management services; case managers develop an individual care plan of services, regularly monitor the plan and modify it as individuals needs change.
- With the onset of case management and home and community based services, seniors in the UWECI service area are remaining independent within their homes an average of 28 months; an average cost savings of $58,000 per senior relative to nursing home care.
We Can Do More: Short Term:
- Increase the number of seniors engaged with case management services by 8%; connecting seniors to services that support them in maintaining their independence.
Long-term:
- Increase the average length of time in which seniors remain independent within their homes to 30 months; an average cost savings of over $63,000 per senior relative to nursing home care.
