George is 75 years old and he lives alone. He suffers from bipolar disorder. His closest living relative is his sister in California.
A social worker from Aging Services completed an in-home assessment with George. She found him very weak and depressed. He was not eating well and his house was cluttered. Referrals were made to Linn County Home Health for assistance with doing laundry, housecleaning and grocery shopping. In addition, arrangements were made for him to receive meals from Meals on Wheels and funding was requested to purchase an emergency response system. The assistance he received allowed George to remain in his own home.
"My case manager is my lifesaver. She helps me take care of myself, while preserving my dignity. I want to remain in my own home but I need a lot of help from others. Without my case manager and the care I receive from Aging Services and other services, I wouldn't have the help I need,” says George.
