Required Documents

  1. Government photo ID for all taxpayers
  2. Social Security card for everyone on the return
  3. Bank account and routing number (for direct deposit/withdrawal). Proof of bank account – routing and account numbers for direct deposit, such as a blank check
  4. Last year’s tax returns or IRS transcript (helpful)
  5. All 2025 tax documents, which might include the following:
    1. Wage and earning statements (Form W-2, W-2G, 1099-NEC) from all employers
    2. Interest and dividend statements from banks (Forms 1099)
    3. 1099-G for unemployment or state tax refunds
    4. Total paid for daycare provider and the daycare provider’s tax identifying number, such as their Social Security Number or business Employer Identification Number
    5. Form 1095-A, Affordable Health Care Statements, if anyone used ACA/Marketplace health insurance in 2025
    6. 1098-T and statement of expenses, if attending college
    7. Any other IRS or state tax documents you received

  3. Enter your information and click the green box at the bottom to finalize your appointment

Where do I go for my appointment?

There will be appointments available throughout Linn and Jones counties.

Do you accept walk-ins?

No, we do not accept walk-ins; you must schedule an appointment with us to have your taxes done.
